Parental agreements with the childcare facility
  • Before medication is dispensed, a parent must provide written authorization including the date, child’s name, medication name, prescription number (if any), dosage, and the dates and times it is to be given. Medication must be in its original container and labeled with the child’s name.
  • A child cannot enter or leave without an escort: a parent, a person authorized by the parent, or facility personnel.
  • Parents are responsible for keeping records current, including changes to phone numbers, work location, emergency contacts, physician, health status, infant feeding plans, and immunization records.
  • The facility will inform parents of incidents involving their child, including illnesses, injuries, and adverse reactions to medication.
  • The facility will obtain written authorization before routine transportation, field trips, special activities away from the center, and water-related activities in water more than two feet deep.
  • The facility will advise parents about their child’s progress, care issues, and individual practices for special needs. Parent participation is encouraged.

Emergency medical authorization and agreement to the center’s policies are recorded in the form below. Final care arrangements and facility approval are completed with the Director.

Transportation & photograph permissions

Transportation

My child may be transported by Auntie Ann’s Daycare to and from off-site activities and field trips.

Photographs

Auntie Ann’s Daycare loves to take pictures of the children while playing and learning. The photographs are posted throughout the daycare, on Auntie Ann’s Daycare Facebook page, and in the local newspaper. Pictures are also used for arts and crafts projects.

Choose Yes or No for each permission in your application. Neither choice is preselected.

Meals, CACFP paperwork & dietary accommodations

Auntie Ann’s Daycare provides healthy meals to enrolled children through the USDA Child and Adult Care Food Program (CACFP). Quality Care for Children is the administrative sponsor.

Packet date: The supplied meal-benefit packet and income chart cover July 1, 2025–June 30, 2026. That eligibility period has ended. Request the current annual packet and income guidelines from the center before applying for meal benefits.

Annual household packet

Each household receives the packet annually, regardless of income. Return completed required forms to the main office.

  • Parent letter and frequently asked questions.
  • Meal Benefit Income Eligibility Statement — return completed.
  • Sharing Information with Medicaid/SCHIP — optional.
  • Infant Affidavit — required for all infants.
  • WIC information and Building for the Future information.

Special meals and dietary accommodations

A Medical Statement to Request Special Meals and/or Accommodations must be completed by a physician or authorized medical authority. A parent note alone does not replace this form. Request it from the center or visit Quality Care for Children forms.

Meal-benefit questions from the supplied packet

  • Use one Income Eligibility Statement for all children in the household attending the same center, and complete all required information.
  • Children in households receiving SNAP, TANF, or FDPIR, foster children, and eligible Head Start participants may qualify for free meals. WIC households may also qualify.
  • Household income determines eligibility for reduced-price meals under the applicable federal guidelines. U.S. citizenship is not required to apply.
  • A household includes people who share income and expenses, including adults and children, relatives, or friends. Foster children may be included; foster-care payments are not counted as household income.
  • Report gross income by source for each household member. Use normal monthly income if the previous month was unusual; include regular overtime rather than occasional overtime.
  • The supplied packet describes benefits as approved for 12 months. Contact the center if income decreases or a job is lost, and request current eligibility guidance.

The online child-enrollment application does not replace the separate annual meal-benefit forms. Provide sensitive benefit information only through the current official forms supplied by the center.

WIC family support

WIC offers nutritious foods, nutrition counseling, breastfeeding support, and healthcare referrals for eligible pregnant, breastfeeding, or recently pregnant women, infants, and children under five. Eligibility considers income and nutritional need. You do not have to receive public assistance to apply. Contact your local health department for current eligibility and food benefits.
